I__________________________________ in the legal age of____ will serve On-tue company and willing to work on any shifting schedule. I will submit the
employment requirements on or before the payout date June 22, 2009 or else i will not be able to receive my salary if without the completion of my requirements.
Since the company doesn't deduct any witholding tax from my salary and no SSS,pag-ibig or Phil health benefits it is my obligation as an employee to pay for my benefit bills
while the company is still planning to enroll for the health insurance to provide better living for the employee of On-tue company.

The contract will ended after 6 months and the employee will be free from their work and the company will? have the right not to rehire the employee.
 

Salary wage:
Terms and Condition:
1. No work no pay.
2. The salary of 7th payment day is for the working days from 16th to the end of the previous month. The salary of 22nd payment day is for the working days from 1st to 15th of that month.
3. If the employee is late he must inform the immediate supervisor.
4. If the employee render absences he must inform 3 hours before his work time.
5. And rest days is Saturday and Sunday.
6. The company may dismiss the employee at any time regarding the status of the business.
7. The employee will be tested every week on their work. If the result is not good, the employee will be dismissed.
Computation:
One Month Salary: 5000php - This will be paid in 2 payout dates, every 7th and 22nd of the month.
For perfect attendance for one month 1500 will be given every 22nd of the month.
