Certificate of Membership/Retirement Plan              11.      Latest Financial Statement certified by an independent CPA              12.      Approved Tuition and Other School Fees              13.      Proposed Curriculum for new courses              14.      List of Faculty, administrative and supervisor staff including educational classifications,                         special training attaching therewith their transcript of records and willingness to join              15.      List of Library Holdings              16.      List of laboratory facilities, equipment, furniture, supplies and materials by subject area              17.      Proposed Budget              18.      School Calendar              19.      Class Program              20.      Teacher’s Program              21.      Teacher’s Contract              22.      Pictures of the school site & facilities              23.      Xerox copy of Latest Government Permit              24.      School Bond (O.R.)              25.      Application / Inspection Fees (O.R.)
